RACI in one page

Responsible: does the work. There can be several. Accountable: owns the outcome and signs it off. Exactly one per row. Consulted: asked for input before the work is done. Informed: told once it is done.

Warning signs: a row with no A, a row with two, a person who is C on everything, and a column so full that one person is holding the whole project.

Responsible (does the work), Accountable (owns the outcome and signs it off), Consulted (asked for input before it is done) and Informed (told once it is done). The distinction between responsible and accountable is the one that prevents most arguments.
It should not. Exactly one accountable person per row is the rule that makes a RACI chart work; two accountable people usually means neither acts, and none means the work waits for somebody to notice.
Keep it to the deliverables and decisions that actually cause confusion, build it in a meeting with the people named in it, and revisit it when somebody joins or leaves. A chart made alone and emailed round is rarely read.
