They are the same kind of pass for different people: Interrail is for residents of Europe, and Eurail is for visitors from outside Europe. Both cover travel on most trains across many European countries.
On many high-speed, international and night trains, yes, and they cost extra on top of the pass. Regional trains usually need none. Checking the route for reservation requirements is one of the first tasks, since popular trains sell out.
Long enough to stay two or three nights in most places. Two to four weeks is common, and trying to see a new city every day tends to turn the trip into a series of train journeys.
